Membership

Who can be a qualified member of ANSAP?

Registered Nurses who or are currently occupying administrative / managerial positions in government or private health care facilities are qualified to be members of the Association. The association shall have regular, life and honorary memberships.

Non Members
A respected members of the community who has rendered meritorious contribution to the Association. - this honorary membership is usually conferred by the BOD based on a set criteria.

Requirements to be submitted:


  • Old/previous membership card (for old member)
  • Certificate of Employment
  • Photocopy of PRC license
  • 2pcs (2x2) recent pictures (white background)
  • 500.00 Pesos processing fee for new/renewal application (regular members)
Regular Members

with an appointment to hold her/his present administrative position.

  • of good moral character
  • currently PRC licensed
  • had paid the prescribed fee for the year.
Requirements to be submitted:

  • Old/previous membership card (for old member)
  • Certificate of Employment
  • Photocopy of PRC license
  • 2pcs (2x2) recent pictures (white background)
  • 500.00 Pesos processing fee for new/renewal application (regular members)
Life Members

must have been a regular member for 5 consecutive years.

  • of good moral character
  • with current PRC License
  • had paid the prescribed lifetime membership fee
Requirements to be submitted:

  • Old/previous membership card (for old member)
  • Certificate of Employment
  • Photocopy of PRC license
  • 2pcs (2x2) recent pictures (white background)
  • 3,000.00 Pesos processing fee for life membership
Providers
Requirements to be submitted:

  • With this application, submit all documents listed below in duplicate copies four (4) weeks before the date of conduction:
    1. Organizational Structure of Nursing Service of the Hospital
    2. Curriculum Vitae of Director of Nursing or its equivalent and other officials of the Nursing Service.
    3. Photocopy of IV Trainer's Card
    4. Photocopy of Preceptor's Card
    5. Affidavit of undertaking
    6. Payment of required fee (non-refundable)
  • An accredited IV Provider acknowledges that the accreditation may be cancelled for any violations of ANSAP rules and regulations, or failure to comply with the aggreements and representations contained in the Standards of IVT.


Chapters
Requirements to be submitted:

  • Application form must be completely accomplished and signed in duplicate
  • With this application, please attached the following documents (checked all documents enclosed):
    • Evidence of Organizational Set-up
    • List of Membership of not less than 50
    • Targeted Plan of activities
    • Evidence of professional and community involvement
    • Copy of your Constitution and By-laws as adopted from ANSAP National Office
  • Payment of initial registration of chapter fee, processing fees, and membership fees.
